Why is the same model always near the top?

Because the sort is honest. Rooms rank purely by live viewer count, and models with big followings pull big crowds every time they stream. Nobody buys the top spot. If the usual names dominate, that reflects real audiences, and it also means the rows below are stacked with less crowded rooms worth opening.
